In this episode, host Alyssa Watson, DVM, is joined by Sara Jablonski, DVM, PhD, DACVIM (SAIM), to talk about her recent Clinician’s Brief article, “Protein-losing Enteropathy in Dogs.” With a career focus on this syndrome, Dr. Jablonski combines evidence and experience to give us the facts, dispel some myths, and equip us all with what’s needed to help our PLE pups.

Key Takeaways
PLE is a syndrome involving excessive protein loss across gut mucosa.
Think about diagnosis in 3 steps: exclude other causes of hypoalbuminemia (eg, renal loss, hepatic insufficiency), determine cause of PLE (eg, inflammatory enteropathy, lymphangiectasia), then screen for associated problems.
Dietary management should be implemented early and often, likely starting with a hydrolyzed low-fat food +/- fiber supplementation, but no one diet fits all.
Steroids can be started right away or added if symptoms are more severe, if inflammatory disease is confirmed, or if diet alone has failed, and 1 mg/kg/day is probably as high as you need to go.
Nonsteroid drugs and supplemental treatments can be used case-by-case, but B12 is safe to replace regardless and there is no evidence of autoimmunity in lymphangiectasia.
